Home owner and payor of premiums is somehow not insured on homeowner policy. Tx

Kind of a weird question. Somehow…a large insurer issued a homeowners policy in the name of the then occupants (as insured) of a home. The occupant has not lived in the home for years and has zero ownership interest in the home (and has never at any time had an interest in the home) Owner is a relative of insured and also on the account but not as insured. Fast forward to a claim related to recent hurricane. Check issued to the then occupants who again, don’t own any interest in the home, and insurer declining to issue check to owner of property. Insurer declining to assist in any way. Any one have any ideas?
